Health Impact Grant Monthly Report: United States

Special Olympics Health is made possible by the Golisano Foundation, and in the United States works in collaboration with the US Centers for Disease Control (CDC).

This monthly report is required for all USA Special Olympics Programs, including Puerto Rico, who have received a CDC-funded Healthy Communities Impact Grant at Tier 1, 2 or 3 level.


Questions are categorized by Prevention, Assessment, Training, and Health Systems, but we recognize that you may not work on all of these every month.


** Reports are due on the 5th of each month. Please report on activities from the previous month’s work. For example: on March 5, you will report on work from February. **

Successes and Challenges

What successes and/or challenges have you had related to Health Pillar 1: Prevention?


Reminder: this encompasses Young Athletes; Family Health Forums; fitness programming and training coaches in fitness; and health education programming.

What successes and/or challenges have you had related to Health Pillar 2: Assessment?


Reminder: Pillar 2 encompasses Healthy Athletes screenings; follow-up care coordination; and partnering with local healthcare providers/organizations to ensure they are trained and accessible to people with IDD (and visible via your service directory).

What successes and/or challenges have you had related to Health Pillar 3: Training?


Reminder: Pillar 3 encompasses training of health professionals and health professional students (both at Healthy Athletes events AND outside of Healthy Athletes), as well as partnerships with health professional schools.

What successes and/or challenges have you had related to Health Pillar 4: Health Systems?


Reminder: Pillar 4 encompasses:


  • training and activation of Health Messengers
  • inclusive health trainings and technical assistance for local and state health agency
  • inclusive health community collaboratives

For Tier 1 or 2 Special Olympics Programs working on the Intellectual Disability Inclusive Health Community Collaborative, please let us know how this project is going.

Impact, Stories, and Requests

What successes and/or challenges have you had related to the Communications goals of your grant (Health-related website or social media posts; interviews with Health Messengers; etc.?)

Please share any stories of athlete leadership, how you’ve engaged athlete leaders and Health Messengers in your project, and any tips or feedback from athletes about your health work.

Please share any stories from the past month about how your Health programming has made a difference in the lives of athlete(s). We read these reports for impact stories to share with our funders, so please include the following details if they are available:


-Athlete name, age, and geographic location


-Athlete’s health status or experience prior to participating in health programming


-The specific programs, screenings, or trainings the athlete engaged in


-The impact on the athlete’s overall health (Please be as detailed as possible: for example, did they walk X number of total miles? Lower their blood pressure by a certain amount? Report an improvement in their mental health and wellbeing?)


-If/how the athlete has engaged other athletes in health as a result.

Please share any images, media, presentations or newsletters that may exist related to any of the work or impact stories described above.


Please clearly name your files when submitting multiple, so that staff can clearly identify them.
